There are four parts. Authority to Represent, Client Statement of Rights, Authorization to Release Financial Information and Contact Information Sheet.
1. AUTHORITY TO REPRESENT – CO-COUNSEL AGREEMENT
There is a place at the end of this section for the name of the business, signature of the authorized representative, date signed and contact information. This document provides the terms by which ERG and Shestokas & Associates represents someone pursuing a claim under the BP Settlement Agreement.
2. STATEMENT OF CLIENT’S RIGHTS
This document describes the rights of a client hiring an attorney. It includes information regarding the three day period during which the client may change his mind and other important information that someone hiring an attorney is entitled to know. At the end there is a place to print, sign and date.
3. CLIENT AUTHORIZATION FOR RELEASE OF FINANCIAL AND TAX RECORDS TO ERG
For clients whose information is kept by other professionals, such as their accountant, this allows the law firm to contact the record keeper directly to obtain the needed information.
4. CLIENT INFORMATION SHEET
This requests relevant contact information for the client, the business and if applicable, the record keeper. There is a section asking about employment information, which only applies in the instance of an individual claim.
